Case Management

Case Management Offices are located in different areas throughout the county and are typically the starting point for services. Case Management is a collection of coordinating, brokering, advocacy, assessment and administrative activities that link the child and/or family with the treatment and support services that they need. All of these services can be accessed by contacting your local case management office (below).

Types of Case Management
Administrative Service Case Management (ACM)
This service links the child and family with services within the Mental Health system as well as other child-serving systems such as education, crisis services, juvenile justice, child welfare, mental retardation, financial entitlements, and others. This position also serves as a resource for the community. Anyone can access this service without cost via a direct phone call/referral.

Blended Case Management
This service consists of a case manager who delivers both Resource Coordination and Intensive Case Management:

  • Resource Coordination (RC)
    This service is available to children with serious emotional disorders to build upon the strengths of the child and family in order to achieve stability within the community. Each RC works with up to 30 children and their families.

  • Intensive Case Management (ICM)
    This service is available to children with serious emotional disorders. ICM provides aggressive and creative strategies in order to gain access to resources and services identified in the treatment or service plan. Each ICM works with no more than 15 children and families in order to assure the level of service needed. Members of the Intensive Case Management Team are available 24 hours, 7 days a week for emergency and crisis situations.
Transitional Case Management (TCM)
This service is available to adolescents between 16 and 22 who have unique needs. This team specializes in issues that face adolescents as they are moving towards adulthood. These issues include: housing, transportation, vocational needs, completing school and graduation, and independent living skills.
