The Montgomery County Office of Developmental Disabilities requires a Psychological Evaluation to determine if an individual is eligible for services.
The evaluation must contain the following information:
A psychological evaluation from a licensed psychologist, certified school psychologist, physician, or psychiatrist with full-scale I.Q. results (70 or below) and a diagnosis of intellectual disability or a diagnosis of Autism.
Evidence of the onset of intellectual disability prior to the individual’s 22nd birthday.
Evidence individual has significant limitation in adaptive functioning in at least two of the following areas: Communication, self-care, home living, social/interpersonal skills, use of community resources, self direction, functional academic skills, work, health, and safety.

Contact the Intake Coordinator at 610-278-5666 to begin the intake process. An intake meeting will be scheduled if the individual meets the eligibility requirements.
An individual will be offered a choice of Supports Coordination providers during the intake meeting. An individual may select any qualified and willing Supports Coordination Organization. The organizations providing supports coordination services to individuals in Montgomery County are:
